Immunization & Medical Entrance Requirements

Children need to receive the minimum number of age appropriate vaccines prior to entering school/preschool. Proof of vaccination MUST be submitted directly to the school nurse NO LATER THAN AUGUST 15.
 

Pre-K Requirements

Minimum Immunization:
  1. DPT-4 doses of DPT. If DPT was given, a medical contraindication letter must be written by your physician on his stationary. This letter will be kept in the student's permanent health records.
  2. Polio – a minimum of 3 doses.
  3. Measles, Mumps, Rubella or MMR – 1 dose of MMR or of each component on or after the 1st birthday.
  4. HIB – 1 dose on or after the 1st birthday.
  5. Varicella (chickenpox) – 1 dose on or after the 1st birthday or a statement of previous varicella disease.
  6. Pneumococcal Conjugate Vaccine (PCV) – 1 does on or after the 1st birthday.
  7. Influenza Vaccine – 1 dose between September 1, 2012 and December 31, 2012.
  8. Proof of Immunization – a copy of your child's immunization records from your physician or local health department.

Kindergarten Requirements

  1. DTaP — 5 doses (4 doses only if the 4th dose received after the 4th birthday)
  2. Polio — 4 doses (3 doses only if the 3rd dose received after the 4th birthday)
  3. Hep B — 3 doses
  4. HIB — 1-4 doses (a minimum of 1 dose on or after the first birthday)
  5. Pneumococcal Conjugate Vaccine (PCV) – 1-4 doses (a minimum of 1 dose on or after the 1st birthday)
  6. MMR – 2 doses
  7. Varicella (chickenpox) – 1-2 doses (a minimum of 1 dose on or after the 1st birthday)
     *  A TB test may be required if a child is transferring from out of state or a high-risk area. Please consult your physician.

Physical Examination

A current physical exam is required for all students entering Reverend Brown School. Please use the PK-4 form below for all students. Students entering Grade 1 or higher for the first time will not be held to having their physical completed between May 15 and August 15 during their first year. Returning students, please schedule your well care visits accordingly.

As of February 1, 2018, potential volunteers must complete VIRTUS registration online. Background check, code of conduct, and sign-up for the VIRTUS classes will be done electronically.
